Illawarra Folk Club Artist Confirmation Form

Please fill in this form if you have been booked to perform at an Illawarra Folk Club concert or event. This will help arrange logistics, accommodation (if required) and promotion of the event.

If you have been offered a fee please confirm the amount here. You will need to provide an Invoice with your ABN or a Statement of Supplier of Form. These forms will be available at the concert.
Give us your best representation of your performance to share.
Provide a URL/Webpage address where we can find a high resolution digital image of the artist. Preferably at least 1MB in size. Alternatively email an image to bookings@illawarrafolkclub.org.au. Name the file with the name of the Artist.
This is to be used for promoting the performance on our website, Facebook. Make it exciting and descriptive of what the artist does!
We usually provide free entry to the artist(s) and their partners. Please supply names to be added to the complimentary entry door list.
Provide some information about sound requiremets for the artist. Include name, instrument, vocal microphone, instrument microphone and DIs required. Eg BRUCE: guitar and mandolin, 1 vocal, 1 instrument mic, 1 DI EMILY: 1 vocal microphone, DI and power for piano
If you have been offered home stay billet accommodation please provide names and bedding arrangement, eg Bruce and Robyn Double, Bill single, Dolly single
Please advise date of arrival and date of departure, Eg Arrive 5 September, depart 6 September. Accommodation is usually provided only for the evening of the performance day.
